We are looking for a highly experienced and exceptionally gifted All Age Continuing Care leader to join our senior management team.
You will have current experience working in at a leadership level in All Age Continuing Care and have a comprehensive understanding of the key working relationships needed to support an effective team. This includes working experience and in-depth knowledge of all areas including children's continuing care, continuing healthcare, s117 aftercare, personal health budgets, commissioning, brokerage and social care / care Act interfaces. You will have excellent interpersonal skills and an inclusive leadership style which is supportive of staff and enabling to support the provision of what is a complex service. Main duties of the job
The All-Age Continuing Care Team supports over 7,500 clients through a range of funding arrangements including Continuing Healthcare, Section 117 aftercare, specialist placements, Children's Continuing Care and other joint funding and oversight of a budget of £392M.

Job responsibilities
Develop the strategy for NHS All Age Continuing Care, supporting and ensuring alignment across the ICB, removing unwarranted variation and promoting innovation in service delivery.
Engage with key strategic regional and national policy makers to inform development of strategy and policies.
Develop and communicate the vision for NHS All Age Continuing Care in the ICB, engaging with key partners and ensure strategy and policies support this vision.
Identify examples of national best practice to ensure that patients benefit from relevant innovations in NHS All Age Continuing Care.
Continue the improvements in NHS All Age Continuing Care to achieve the standards required in the National Framework NHSE Assurance Framework, S117 of the Mental Health Act.
Drive the transformation of NHS All Age Continuing Care in the ICB to meet the aspirations of the ICB to have a Place-based integrated delivery model that supports joint working with local teams and services.

Knowledge, Training and Experience
Essential
- Professional Registration for the Clinical Delivery arm of AACC - nurse, AHP or social worker.
- Educated to master's level or equivalent level of experience of working at a senior level in specialist area.
- Proven and significant leadership and management experience in a health or social care environment.
- Proven experience of personally achieving board approvals and leading and delivering complex change programmes in a politically sensitive and complex environment.
- Commissioning expertise in healthcare environment.
- Significant experience and understanding of proven implementation of project management methodologies.
- Experience of successfully operating in and delivering priorities in a partnership environment.
- Extensive Knowledge of NHS All Age Continuing Care.
- Evidence of post qualifying and continuing professional development.
- Must have an understanding of the background to and aims of current health and social care policy and appreciate the implications of this on engagement.
Desirable
- Should have an appreciation of the relationship between the public sector organisations that interface with NHS All Age Continuing Care including the independent market and commissioning organisations.
