Fixed-term contract until 31 March 2028 | 16 hours per week | Salary dependent on experience

We are looking for a caring, organised and proactive Frailty Care Coordinator to join our team at Shap Medical Practice on a fixed-term basis until 31 March 2028, working 16 hours per week. Salary will be dependent on experience.

We are ideally looking for someone with a background in healthcare, social care or the third sector, although we are also interested in people who can demonstrate the right skills, experience and approach. Most importantly, we are looking for someone who genuinely cares about supporting our frail and older patients and who is able to build positive, trusting relationships with patients, their families and the wider team.

The postholder will work closely with GPs, nurses and the wider multidisciplinary team to manage a caseload of patients, helping to identify needs, coordinate appropriate support and ensure that patients are connected with the services and organisations that can help them.

This is an opportunity to make a real difference to patients in our rural community by helping people stay well, independent and supported in their own homes for as long as possible.

Main duties of the job

The role will play an important part in proactively identifying and supporting people living with frailty, particularly older people and those with complex health and social care needs. The postholder will provide care coordination, assessment and navigation across health, care and community services, helping people to remain as independent as possible and supporting admission avoidance and appropriate care in the community.

Shap Medical Practice is a friendly, rural GP practice serving around 3,300 patients across Shap and the surrounding communities. We are a close-knit team who work together to provide high-quality care to our patients and support one another in what can be a busy and varied environment.

Our team includes GPs, nurses, healthcare assistants, dispensary staff, administrators and reception colleagues, alongside a wider multidisciplinary team and community services. We value good communication, teamwork and a willingness to help each other out.

We are looking for someone who will fit well within our team someone who is approachable, compassionate and organised, but who can also work independently and take ownership of their role. The successful candidate will have the opportunity to build strong relationships with patients and colleagues and to become an important part of the way we support our older and more vulnerable patients.

Job responsibilities

The successful candidate will be caring, dedicated, reliable and person-focused, with an interest in working with older people and people living with frailty. They will have good communication, organisational and time management skills, be able to work autonomously and as part of a multidisciplinary team, and be willing to develop their knowledge and clinical skills.
