Job summary
The Occupational Therapist in the Long Term OT Team, will support adults with complex, long-term conditions and disabilities to live safely and independently in their own homes.
This role holder will manage a specialist caseload, completing person-centred, strengths-based assessments and delivering practical interventions tailored to individual goals.
This includes prescribing and monitoring equipment, promoting assistive technology, supporting safe transfers of care and maintaining accurate clinical records. Working collaboratively with service users, carers and partners across health, housing, social care and the voluntary sector, you will contribute to coordinated care, service improvement and high-quality professional practice, while providing guidance and supervision to Occupational Therapy Assistants where appropriate.

Conduct holistic, person centred assessments of service users and carers in their homes.
Manage a specialist caseload involving adults with complex, long term conditions and disabilities.
Develop strengths-based support plans and set SMART therapeutic goals.

- Pay Level: 7, £43,253 - £48,795 (fully inclusive of Local Weighting Allowance)

- Location: Observatory House/ Slough Hybrid (minimum 2 days per week in the office)

What you’ll be doing:
We are hiring 2 experienced Occupational Therapist looking to make a meaningful impact on their community. This is your opportunity to join a dynamic and supportive Long Term Occupational Therapy Team at Slough Borough Council, where your expertise will directly contribute to improving the lives of adults with complex and long-term needs.
As a Specialist Occupational Therapist, you’ll work alongside multidisciplinary teams, delivering high-quality, person-centered interventions that promote independence and reduce reliance on long-term care. You’ll play a key role in supporting individuals with physical and mental health conditions, learning disabilities, and age-related challenges, helping them live safely and confidently in their own homes.
You’ll also contribute to exciting and innovative projects, including:
- Leading on Disabled Facilities Grant (DFG) referrals and environmental adaptations.
- Promoting the use of Assistive Technology and digital tools like AskSARA to enhance independence.
- Supporting Discharge to Assess pathways, Proportionate Care and In-Reach projects with residential settings and Domiciliary Care Providers.
- Collaborating with Integrated Care and Decision Making (ICDM) clusters, GPs, and community health professionals.

Annual increments, where applicable, are usually payable on 1 April, subject to six months’ service and satisfactory performance, until the maximum spinal column point is reached.

- Can demonstrate and evidence skills in working with all age groups and many pathologies and disabilities
- Experience of working with people of diverse cultural and religious communities
- Proven ability in implementing Clinical Governance.
- Experience of contributing to service/quality improvement.
- Experience of working within a community or acute therapy team.
- Experience of case-management in clients with complex long-term conditions and disabilities
- Experience of working with staff and managers at all levels across the community, engaging with health, housing, partner agencies, and voluntary sector.
- Moving and Handling case work experience.
- Experience of diffusing conflict and complaints.
