The DM01 Diagnostics Pathway Coordinator will play a key role in supporting the effective management of diagnostic patient pathways and waiting lists. The post holder will be responsible for validating and tracking patients throughout their diagnostic journey, ensuring appointments are managed in line with national DM01 standards and the Trust's operational requirements.

Working collaboratively with consultants, physiologists, booking teams and the wider multidisciplinary team, the post holder will proactively manage waiting lists, ensure patients are booked within the 6-week diagnostic standard, monitor patients approaching breach, and take appropriate action to maintain compliance. The role includes identifying and escalating capacity issues, supporting waiting list validation, preparing performance information, and contributing to weekly PTL meetings.

Main duties of the job

Validate and manage the DM01 diagnostic waiting list Ensure patients are booked within the 6-week diagnostic booking window Monitor patients approaching breaches and take appropriate action Reschedule appointments proactively to optimise capacity and maintain compliance Escalate capacity concerns, pathway delays, and breach risks Track patient pathways from referral to diagnostic test completion Attend and contribute to weekly PTL meetings Ensure clinics and diagnostic sessions are fully utilised and flag unused capacity.

Job responsibilities

The fundamental role of the Pathway Co-ordinator is to ensure that patients are being managed safely on their pathway from referral to investigation, ensuring all relevant information is available and to record information relating to clinical episodes on relevant IT systems. The post holder will have specific responsibilities for the prospective monitoring of access targets and will escalate potential breaches to the Service Manager.

Qualifications

Essential

  • Educated to GCSE level including English or equivalent qualification
  • Computer literate including ability to use Microsoft packages such as Excel
Knowledge

Essential

  • Understanding of national NHS targets
  • Understanding of Cardiology databases such as Solus, Pacenet and Change Healthcare
  • Be able to use PAS, EPR, SOLUS and other patient systems
Experience

Essential

  • Relevant administrative experience within a hospital / healthcare setting
  • Cardiology experience and knowledge of technical terminology and IT systems, preferably device and CDC experience
Skills

Essential

  • Excellent attention to detail, with the ability to accurately maintain DM01 activity, monitor patient pathways, and manage waiting lists.
