Practice Nurse

Job summary
As an integral member of our clinical team, you will provide evidence-based nursing care and support a wide range of chronic disease management and preventative health services. You will work closely with GPs, healthcare assistant, pharmacists, and administrative staff to deliver outstanding patient-centred care.
Main duties of the job
- Chronic disease management including asthma, COPD, diabetes and cardiovascular disease reviews
- Cervical screening and womens health services
- Childhood and adult immunisations and vaccinations
- Travel health assessments and vaccinations
- Wound care, dressings and suture removal
- Health promotion and lifestyle advice
- Phlebotomy, blood pressure monitoring and other clinical procedures
- Supporting quality improvement initiatives and achievement of QOF targets
- Maintaining accurate clinical records and adhering to professional standards
- The successful candidate will
- Be a registered nurse with current NMC registration
- Have previous experience as a Practice Nurse working within primary care
- Demonstrate competence in chronic disease management
- Be experienced in cervical cytology and immunisations
- Possess excellent communication and interpersonal skills
- Be able to work independently whilst contributing effectively as part of a multidisciplinary team
- Show a commitment to continuous professional development and high standards of patient care
- Infection control lead

Qualifications
Essential
- Be a registered nurse with current NMC registration
- Be experienced in cervical cytology and immunisations
Desirable
- Independent prescribing qualification
- Asthma and/or COPD diploma
- Diabetes qualification
- Travel health qualification
