Job summary

As a Senior Case Worker, you will support adults with care and support needs to improve their health, wellbeing and independence through strengths-based reviews and personalised smoking cessation support. Working with people who may face barriers to accessing mainstream services, including those with disabilities, long-term conditions, mental health needs and complex social circumstances, you'll build trusted relationships and empower individuals to make positive, lasting changes that support a healthier future.

Working within the Younger Adults Team, you'll collaborate with Social Workers, Public Health colleagues and partner organisations to embed smoking cessation into social care practice. Through compassionate, non-judgemental conversations, tailored interventions and effective partnership working, you'll help individuals understand the impact of smoking on their health and wellbeing, access stop smoking services and wider community support, and contribute to the ambitions of Smokefree Hampshire 2030.

Main duties of the job

  • Understanding of the impact of smoking on health, wellbeing, quality of life and wider associated risks.
  • Knowledge of the Care Act 2014, Mental Capacity Act 2005 and how strengths-based practice can support positive outcomes and greater independence.
  • Commitment to promoting positive health and wellbeing, championing smokefree lifestyles and empowering people to make informed decisions about their health.
  • You will be required to travel to various locations within Hampshire and the Isle of Wight or other local authority, and it is essential that you have a current drivers licence and access to a reliable vehicle on a regular basis.
