Job summary
We are seeking an enthusiastic and highly motivated Children's Continuing Care Nurse to join our Children's Continuing Care Team. This is an exciting opportunity to work alongside children, young people and their families to ensure they receive the right support, at the right time, in the most appropriate setting.
The successful candidate will undertake holistic assessments, coordinate care packages, work collaboratively with health, education and social care professionals, and ensure that care provision is person-centred, safe and outcome-focused. The role requires excellent communication, clinical assessment and case management skills, together with a commitment to delivering high-quality care and improving outcomes for children and families.
The job provides the opportunity to hybrid work with a mixture of working from home and in the office. You will primarily cover the Doncaster and Rotherham locality however there may be times it is required to cover the South Yorkshire ICB CCC team.
Main duties of the job
Key Responsibilities
- Undertake Children's Continuing Care assessments and reviews.
- Coordinate and manage a caseload of children and young people with complex health needs.
- Work in partnership with families, providers and multi-agency professionals.
- Develop, review and monitor care packages to ensure assessed needs are met.
- Contribute to safeguarding processes and risk management.
- Prepare clear, evidence-based reports and recommendations for panel consideration.
- Support quality improvement initiatives and service development.

Qualifications
Essential
- Registered Nurse (RN Child/Learning Disability or RSCN or RGN/HV/SCHPN with relevant post registration experience of working with children and young people with identified health needs
- Educated to degree level in relevant subject or an equivalent level of experience working at a similar level
- Current registration with NMC
- Demonstrate relevant post registration experience as a registered nurse with some knowledge of care management or transferrable skills. Experience of working with the multi-disciplinary team and with a wide range of health and social organisations.
- Ability to demonstrate a commitment to professional development.
Desirable
- Relevant Post Basic Registration Course
- Teaching, Mentoring and Assessors qualification
Knowledge and Understanding
Essential
- Must demonstrate an excellent knowledge and understanding of the care needs presented by individuals with a variety of conditions and disabilities.
- Must have a working knowledge and experience of current government legislation around safeguarding strategy and the process of implementing this.
- Able to deliver against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines.
- Able to demonstrate clear communication skills with excellent report writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences.
- Communication skills for nurturing key relationships and maintaining networks. An ability to communicate on complex and sensitive matters and in potentially difficult situations that may require persuasion and influence, at times an ability to convey compassion and understanding to the individual circumstance.
- An ability to analyse and interpret information, to pre-empt and evaluate issues, and recommend an appropriate course of action to address the issues.
- Attention to detail combined with the ability to extract key messages from analysis of complex information
- An autonomous practitioner with demonstrated good judgement, problem-solving and analytical skills.
