Job summary

Are you passionate about delivering community-based therapy, allowing service users to improve their quality of life? Are you committed to a Home First/ pathway 1 ethos, where people can rehabilitate in their own environment?

Our therapists support hospital discharges, admission avoidance and long term self-management, enabling patients to live as independently as possible in their home environment. We support rehabilitation of patients in their place of residence to maximise functional potential including the provision of equipment and liaison with other services as needed. The role includes assessment of functional ability and care support needs, both for establishing rehabilitation goals and longer term care needs if required.

We are seeking a motivated Senior Occupational Therapist to join our busy and dynamic team and promote choice for our service users and their carers. You will be working alongside PT's, case managers and support workers and will develop overlapping skills with these professions.

In return, we'll give you the opportunity to develop skills and enhance your multidisciplinary working; we will provide you with regular supervision as well as the opportunity to develop your leadership skills.

We offer a flexible service which covers extended hours operating 7 days a week, to support our service user needs. Working arrangements include weekends and bank holidays on a rota basis.

Main duties of the job

  • Deliver holistic assessments, planning and evaluating care for patients with acute and complex needs
  • Use clinical reasoning to provide evidence-based, person-centred interventions that promote safe, high-quality care
  • Manage your own caseload and support effective team coordination and service delivery
  • Work proactively with MDT colleagues to prevent avoidable admissions and support timely discharges
  • Collaborate with patients, families and carers to set meaningful goals that maximise independence and quality of life
  • Facilitate referrals and coordinate care based on individual needs
  • Assess and arrange appropriate equipment within scope of practice
  • Support, supervise and contribute to the development of junior staff, peers and students
  • Maintain accurate, timely and legally compliant records
  • Contribute to training and continuous service improvement
  • Identify and manage risk to ensure the safety of patients, colleagues and yourself

Essential

  • BSc or equivalent in Occupational Therapy and HCPC registration
  • Evidence of continuing professional development and commitment to lifelong learning

Desirable

  • Member of the College of Occupational Therapists
Skills, knowledge and experience

Essential

  • Explain and evidence how you have effectively led a team with senior support when required
  • Explain and evidence how you've made clinically reasoned judgements when working with a variety of conditions
  • Explain and give evidence of supervising staff and students of own/ other profession
  • Demonstrate effective time management and caseload management with a community caseload
  • Explain and evidence how you're able to work autonomously, with the ability to know when to seek appropriate advice / guidance from senior staff.

Desirable

  • Recent relevant experience within the community setting
