Job summary
We are currently looking for a dynamic and enthusiastic Senior Physiotherapist to join our expanding Intermediate Care Team.
The vision of the service is to provide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital.
The post holder will be responsible for ensuring that patients get the best care possible in their own homes, maximising potential in the initial phases upon discharge from hospital in our short stay service. The service also provides therapy within our interim bed environment and supports the prevention of hospital admissions.
Successful applicants will be expected to assist in the development of therapy assistant practitioners and integrated care workers, to support the clinical delivery of the service and proactively work alongside the wider multidisciplinary team to ensure patient flow and safety.

Main duties of the job
To undertake assessment of patients with complex and multiple pathologies using specialist clinical reasoning skills.
To identify patient needs, agree goals and provide appropriate therapy interventions, provide equipment and rehabilitation programmes.
To work across Pathway 1 (care at home) and Pathway 2 (care home beds) for the Intermediate Care service.
To work as an autonomous practitioner within the integrated multi-professional team.
To be responsible for patients on shared caseloads within community and/or care home settings.
To take a holistic approach to all therapeutic interventions, working across professional boundaries at times.

Job responsibilities
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Person specification
Education / Qualifications
Essential
- Degree or equivalent eg. BSc, MSc or Diploma entry level qualification into the Physiotherapy profession
- Current HCPC registration
- Documented evidence of own continuing professional development
Desirable
- Member of CSP
- Fieldwork/Practice Educator qualification
Experience
Essential
- Relevant post-graduate clinical experience in a variety of settings
Desirable
- Community experience
- Equipment knowledge and provision
Skills & Abilities

- Able to demonstrate commitment to high quality care and service provision
