Job summary

An exciting opportunity has arisen for an enthusiastic and motivated Qualified Registered General Nurse with experience in clinical leadership has become available within the Healthcare Department at HMP Manchester. The Senior Practitioner role is within the Primary Care Team and will include:

1. The lead clinical role in identified clinical pathways within the service specification

2. Manage and evaluate relevant clinical pathways within the service specification and will include:

  • Reception screening, process and monitoring
  • Patient discharge and transfer clinic
  • Social Care Team
  • Complex case lead
  • Infection Prevention Lead for Healthcare including medication administration points
  • Equality, Diversion and Equality Lead
  • Provide training to the team in the form of train the trainer model
  • Provide cover for the Primary Care Team Manager in his/her absence and take on any delegated management duties as required. And to participate in the on-call rota.

Main duties of the job

Social care-line management of staff and manage pathway as a whole-be point of contact for integrated neighbourhood team initially-enquires etc...

Discharge and transfer clinic-need guidelines for this, then to monitor process and review

Complex case meetings Monday's chair and ensure documentation of meetings are to a good standard -admin support and uploaded to patients' clinical records-review of these care plans

Cover primary care team manager and long-term conditions nurse when on leave

Weekly clinic activity meeting-follow up actions and support staff

Infection prevention lead

Equality, Diversity and Inclusion lead

In numbers morning and 4pm meds should need to be

Delivery and upskilling staff training when required along others
