Job summary

The Calderdale Community Rehabilitation Team provides assessment, treatment and advice to adults with a rehabilitation need and goal, with staff and service users working together to achieve patient-centred goals. The service comprises 3 geographically based teams within the Calderdale area.

The service manages patients who may have complex neurological conditions such as Motor Neurone Disease and Stroke, acquired brain injury, long term neuro-degenerative conditions, patients with acute and chronic musculoskeletal problems, fallers, frailty, orthopaedic conditions including post-surgery, and complex multi pathology conditions which may include patients with dementia or learning disabilities.

The majority of clinical work takes place within patient's homes (including care homes). However, we also see patients in clinics or groups when it is more appropriate to do so. At times, the team visits patients in hospital to participate in meetings, or to assist complex discharge planning.

QUALIFICATIONS / TRAINING

Essential

  • Degree/Diploma/MSc in Occupational Therapy
  • HCPC Registered
  • Trained Clinical educator
  • Committed to undertaking further training to develop practice, including HEI and M level study

Desirable

  • Skill-sharing/blurred boundary competencies
  • Masters level qualifications in specialist area
KNOWLEDGE, EXPERIENCE & EXPERTISE

Essential

  • Recent experience demonstrating ability to assess, treat and evaluate care at a specialist level in rehabilitation and Long-Term Conditions
  • Experience developing, supervising, appraising and training others
  • Proven ability in implementing Clinical Governance
  • Knowledge of relevant legislation, national guidelines and best-practice for a range of conditions
  • Comprehensive knowledge of appropriate and available Occupational therapy assessments and approaches, including outcome measures and goal setting.
  • Experience of diffusing conflict and complaints
  • Problems solving/decision making skills based on patient/client needs and goals
  • Ability to case manage patients with long term conditions/multi pathologies
  • Experience of contributing to service/quality improvement

Desirable

  • Experience of community care
  • Experience of neurological splinting and orthotics
  • Experience of cognitive therapy
