Job summary

Are you an enthusiastic Speech and Language Therapist passionate about developing your knowledge and skills in providing Community Speech and Language Therapy for Adults with acquired swallowing and communication difficulties?

We have an exciting opportunity for a fixed term Band 5 post beginning in the Community Integrated Neuro and Stroke Service (CINSS) with potential to rotate into the Adult Community SLT Team in the future.

The CINSS Team is a community based interdisciplinary therapy team, supporting the timely discharge of patients from hospital with ongoing needs following a new or worsening neurological condition; stroke, and non-stroke. The team works collaboratively with acute services, rehab and third sector services to deliver a patient centred; goal focused approach to rehabilitation. Our team is an integral part of the stroke pathway.

The Adult Community SLT Team is a small, dedicated team providing assessment, advice and treatment to adults with acquired swallowing and communication difficulties. You will have the opportunity to work with SLTs specialising in progressive neurology, voice and dysfluency. Offering home visits, clinic-based appointments and attendance at clinics at UHCW.

This post will offer you the opportunity to consolidate and develop clinical skills, as well as providing opportunities to be part of quality improvement projects, service developments, working closely with other SLTs and the wider team.

Main duties of the job

To provide speech and language therapy assessment, treatments and review for patients referred into the services with acquired communication, voice and swallowing needs. This will be for patients predominantly in their place of residence but will include clinic and other settings.

To provide expert advice and guidance to patients, carers, health and social care professionals from across multiple providers internal and external to the organisation.

We will support your development through Band 5 NQP and dysphagia competencies. You will have access to Value Based Appraisal, one-to-one and group clinical supervision.

Job responsibilities

  • To be an integral part of the Community Integrated Neuro and Stroke Service/Adult Community SLT service.
  • To provide evidenced based specialist therapy assessments assessment and appropriate therapy intervention to patients referred to the service.
  • Follow defined stroke and neurological care pathways and other relevant rehabilitation pathways as directed by line management.
  • To liaise closely with members of the interdisciplinary team and wider multidisciplinary team as required, facilitating effective caseload management.
  • To utilise specialist skill and knowledge to manage a neurological caseload.
  • To have general knowledge of a variety of conditions including neurological, care of the elderly, understanding the impact of multi pathological presentation.
  • To utilise a comprehensive range of evidenced based specialist assessments and treatment to patients in their own home or in a clinic environment.
  • To assist in the quality and activity monitoring systems, to support evaluation of effectiveness of the service.
  • To assist in the maintenance and development of the service.

Experience

  • oEvidence of good generalist skills. oAware of health and safety requirements. oEvidence of continuing professional development oExperience of working with patients with communication difficulties.

Desirable

  • oWorking in a multi-disciplinary team oExperience of working with patients who have a neurological diagnosis. oEvidence of working with patients with eating, drinking and swallowing difficulties. oRelevant post graduate clinical experience in the United Kingdom with good basic training/post graduate rotations (or within a community/intermediate care setting) oExperience of assisting in audit or research oExperience of working within a community environment oExperience as an SLT or SLTA
