Job summary

Shore Medical is seeking an experienced Advanced Clinical Practitioner (ACP) to join our primary care team, working across our practice sites as required.

The post holder will provide expert clinical assessment, diagnosis and treatment for both adult and paediatric patients presenting with acute and chronic conditions, working within their scope of practice and the four pillars of Advanced Practice. They will provide appropriate management, prescribing and referral where necessary, ensuring patient safety and adherence to national and local guidance.

We are looking for an experienced ACP with a background in Primary Care who is committed to delivering safe, high-quality patient care and maintaining excellent clinical standards. The successful candidate will work collaboratively with the wider multidisciplinary team, supporting clinical leadership and the delivery of effective patient-centred care across Shore Medical.

Main duties of the job

  • Provide autonomous clinical assessment, diagnosis and treatment for adult and paediatric patients with acute, chronic and undifferentiated presentations.
  • Work within the Four Pillars of Advanced Practice, NMC Code and professional scope, ensuring safe, evidence-based and high-quality care.
  • Undertake clinical decision-making, prescribing, investigations and referrals where appropriate.
  • Provide clinical leadership, supervision and support to the wider nursing and multidisciplinary team.
  • Contribute to clinical governance, audit, research and continuous professional development.

    Job responsibilities

    • Demonstrate the ability to work within the Four Pillars of Advanced Practice, (HEE 2017), and the NMC Code of Conduct (2018), and deliver nursing care of the highest standard.
    • Be professionally and legally accountable for all aspects of your work, promoting high standards of clinical care.
    • Provide a first point of contact for patients that present with undifferentiated, undiagnosed problems, making use of skills of history taking, complex clinical decision making to establish diagnosis and management plan.
    • Instigate necessary invasive and non-invasive diagnostic tests and investigations, interpreting findings where appropriate.
    • Prescribe safely, using evidenced based and cost-effective measures as defined by current legislative and professional frameworks within scope of practice, Complete Annual Prescribing Portfolio as part of CPD.

    Person specification

    Qualifications

    Essential

    • Registered Nurse MSc in advanced clinical practice or NHS digital badge Non medical independent prescribing qualification V300 or equivalent Teaching and Assessing qualification.
    • Non-Medical prescribing qualification
    • Full UK Driving Licence (and access to a car)

    Desirable

    • Evidence of ongoing learning, reflection and professional development Recognised Leadership course
    Experience

    Essential

    • Experience of working collaboratively with other agencies.
    • Experience of working with individuals in a Primary care Role
    • Experience of managing own time and diary
    • Experience of mentoring students
    • Experience of autonomous practice
