Job summary

The post-holder is responsible for the ongoing assessment of care needs and the development, implementation and evaluation of programmes of care. The post holder will carry out all relevant forms of care and will take responsibility for caseload holders in their absence

Main duties of the job

Developing and maintaining effective communication networks with other health professionals, statutory and voluntary agencies.

Accurately and timely recording of all care given to the patients and report any changes in the patient's condition to the general practitioner or other members of the health / social care team and that may be used in enquiries and / or court sessions.

Working within the primary care setting as a member of the health care team, responding to End of Life patients. To have a rapid response for patients at End of Life. To work with District Nursing and other stakeholders to support End of Life Care.

Providing and maintaining a high standard of skilled nursing care for patients in their homes, using a nursing model of care, that is consistent with best evidence, within own scope of practice and legislation.

Qualifications & Education

Essential

  • First level registered Nurse
  • Qualification in palliative/ End of Life care or relevant experience
  • Community Experience
  • Evidence of continual professional development
  • Clear understanding of end of life care
  • CEDA form to ensure patient have DNAR/CPR/ACP/ priorities of care in place

Desirable

  • V300 Nurse Prescriber
Knowledge and Training

Essential

  • Demonstrate knowledge of research based practice
  • Good background knowledge of issues within the primary care arena

Desirable

  • Have knowledge of major NHS issues
  • Knowledge around stock requisition and IB Solutions system
Experience

Essential

  • Evidence of being able to work as part of a team
  • Evidence of working to and achieving defined objectives
