Job summary

Working with a supportive community nursing team, you'll provide high-quality care to people in their own homes so they can stay as independent, safe and well as possible. Based on the role of Community Staff Nurse, you will assess needs, plan care and build trusted relationships with patients, families and other health and care professionals. You'll play an important part in delivering person-centred care, supporting people with a wide range of health needs. The role offers opportunities to develop your clinical skills, work autonomously and contribute to service improvement while being supported by experienced colleagues within the wider community nursing team.

Main duties of the job

What you'll do:
  • Carry out holistic assessments and contribute to personalised care planning
  • Deliver safe, evidence-based nursing care in patients' homes
  • Work closely with multidisciplinary colleagues and partner organisations
  • Maintain accurate patient records and support high standards of care
  • Communicate sensitively with patients, families and carers, including in complex situations.
What we're looking for:
  • A Registered Nurse qualification or nearing first-level registration
  • Understanding of community nursing and person-centred care
  • Experience working in a healthcare setting and as part of a team
  • Good communication, assessment and basic IT skills
  • A commitment to learning, developing and delivering compassionate care.

Essential

  • Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration

  • Demonstrate an understanding of the role of a community nurse
  • Knowledge of legislation impacting on service delivery
  • Knowledge of the modernisation agenda
  • Demonstrate an understanding of evidence based practice
  • Knowledge of local initiatives
  • Demonstrate an understanding of the MDT
  • Basic IT skills
  • Knowledge and skills in the assessment process
