Job summary

Working with the District Nursing team, you'll provide high-quality care in people's homes so that they can receive safe, effective support and maintain their independence. Based on the role outlined inthis position plays an important part in supporting patients with a wide range of health needs and ensuring care is tailored to their individual circumstances. You'll work closely with patients, families, carers and a range of health and care professionals to assess needs, plan care and deliver evidence-based nursing interventions. The role offers opportunities to develop your skills, work autonomously and contribute to service improvement within a supportive multidisciplinary team.

Main duties of the job

What you'll do:
  • Assess patients' needs and develop personalised care plans
  • Deliver a range of nursing interventions and treatments safely and effectively
  • Work with patients, carers and partner organisations to coordinate care
  • Maintain accurate clinical records and promote high standards of care
  • Support service improvements and contribute to the development of colleagues and learners
What we're looking for:
  • Registered Nurse qualification and current professional registration
  • Understanding of community nursing, patient assessment and evidence-based practice
  • Experience working within a healthcare setting and as part of a team
  • Strong communication skills and the ability to handle sensitive conversations with care
  • Commitment to learning, professional development and delivering compassionate care

Essential

  • Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration

  • Post-registration qualifications
  • Mentorship qualification
Knowledge

Essential

  • Demonstrate an understanding of the role of a community nurse
  • Knowledge of legislation impacting on service delivery
  • Knowledge of the modernisation agenda
  • Demonstrate an understanding of evidence based practice
  • Knowledge of local initiatives
  • Demonstrate an understanding of the MDT
  • Basic IT skills
  • Knowledge and skills in the assessment process

Desirable

  • Able to mentor students
  • Evidence of professional development
  • Have a working knowledge and understanding of First Visit Criteria
Experience

  • Basic health and safety awareness
  • Experience of working within a healthcare setting
  • Experience of working with families and carers
