Job summary

You'll support people in their own homes through skilled nursing care, helping them stay safe, independent and well while receiving the treatment they need. The Community Staff Nurse plays an important role within the District Nursing Service, delivering high-quality care and building positive relationships with patients, families and partner organisations. Working as part of a supportive multidisciplinary team, you will use your clinical judgement to assess needs, plan care and respond to changing circumstances.

Main duties of the job

What you'll do:
  • Carry out holistic assessments and develop individualised care plans for patients with a range of health needs
  • Deliver evidence-based nursing care, including a variety of clinical interventions within your scope of practice
  • Work closely with patients, carers and health professionals to ensure coordinated care and positive outcomes
  • Maintain accurate clinical records and contribute to safe, effective service delivery
  • Support service improvement, learning and the development of students and junior colleagues
What we're looking for:
  • Registered Nurse qualification and current professional registration
  • Experience working in a healthcare setting and as part of a team
  • Strong communication, assessment and clinical decision-making skills
  • A caring, flexible approach and commitment to ongoing learning and development
  • A full UK driving licence and access to a vehicle for work purposes

  • Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration

  • Demonstrate an understanding of the role of a community nurse
  • Knowledge of legislation impacting on service delivery
  • Knowledge of the modernisation agenda
  • Demonstrate an understanding of evidence based practice
  • Knowledge of local initiatives
  • Demonstrate an understanding of the MDT
  • Basic IT skills
  • Knowledge and skills in the assessment process
