We are looking for people who wish to develop into senior specialist diabetesnurses, working across primary, community and specialist secondary care services, including a large technology service, who have experience working in diabetes and which to progress their career further.
Main duties of the job
The post holder will work as part of the diabetes integrated multi-disciplinary team providing clinical support to community based intermediate diabetes care clinics, specialist diabetes clinics, and the in-patient population with diabetes. The post-holder will be responsible for the management of a defined caseload of patients with Diabetes, providing specialist nursing support, advice, education and/or care to that specific group. They will also participate in the delivery of structured patient diabetes education sessions and healthcare professional education both for community and hospital-based staff.
The post holder will work as part of innovative diabetes nursing team, supporting the implementation of Think Glucose, a national initiative designed to improve the care of in-patients with diabetes, across Bradford Teaching Hospitals Foundation Trust (BTHFT)

Experience
Essential
- Significant post-registration experience, including experience of working in within Diabetes
- Experience of participating in clinical audit
Desirable
- Experience of developing innovative practice
- Experience of working in an MDT and across organisational and professional boundaries
Skills
Essential
- Report writing / Presentation skills
- Computer literate - able to use Microsoft suite and clinical systems
- Clinical skills specific to area of specialty
- Ability to work as part of a team
- Organised approach to work with ability to prioritise and delegate
- Mentoring and advanced communication skills
- Leadership skills
- Excellent time management and organisation skills
- Proven skills in teaching/facilitating individuals and small groups
- Evidence of reflective practice

- Proven evidence of change management skills
Knowledge
Essential
- Understanding of Information Governance and Confidentiality
- Knowledge and understanding of application of relevant National Guidance both general and specialty related e.g. National Service Frameworks, NICE etc.
- Knowledge and understanding of the NHS national targets and priorities

- Recruitment and Selection, Appraisal and other relevant HR/People management training
- Knowledge of change management processes
Qualifications
Essential
- Appropriate NMC Registration (RN1)
- Degree or equivalent qualification or experience
- Evidence of current CPD
Desirable
- Post registration qualification in diabetes
- Leadership or management development qualification
- Non-medical prescribing qualification (V300)
