Job summary

The Learning Disability Acute Liaison Nurse will work within the Complex Needs Team and provide a specialist service across York and Scarborough acute hospitals, supporting people with learning disabilities and their carers to access planned, unplanned and day-treatment healthcare services. The post holder manages a defined caseload, assesses care needs, coordinates complex care planning, and ensures reasonable adjustments are implemented to improve patient experience and outcomes. The role also provides specialist advice regarding the Mental Capacity Act, consent, best interests decision-making, discharge planning and accessible healthcare information. The post holder works collaboratively with acute services, community learning disability teams and external partners to improve access, quality and equity of care for people with learning disabilities.

Main duties of the job

Manage a defined caseload of patients with learning disabilities accessing acute healthcare services.

Assess, plan, implement and evaluate specialist care and treatment programmes.

Coordinate complex patient pathways, including admission, treatment and discharge planning.

Provide specialist advice regarding reasonable adjustments, consent, best interests decisions and the Mental Capacity Act.

Act as a specialist point of contact for patients, carers and professionals.

Support patients and carers to understand healthcare information through accessible communication.

Identify and address barriers to accessing acute healthcare services.

Deliver learning disability awareness training and education programmes.

Work closely with multidisciplinary teams and community services to ensure seamless care.

Contribute to service development, policy implementation, audit and quality improvement activities.

Support the development of learning disability liaison services across the Trust.

Maintain accurate records and comply with professional and organisational standards.

Person specification

Experiance and Knowledge

Essential

  • oRegistered Nurse with experience working with people with learning disabilities.
  • oExperience working at Band 5 level or equivalent within a relevant healthcare setting.
  • oExperience of assessment, care planning, implementation and evaluation of care.
  • oKnowledge of current issues affecting learning disability healthcare.
  • oUnderstanding of the Mental Capacity Act and consent processes.
  • oUnderstanding of local and national learning disability agendas and relevant national guidance.
  • oExperience of multidisciplinary working and collaborative care coordination.
  • oKnowledge of audit, quality improvement and evidence-based practice.
  • oExperience delivering training, education or awareness sessions.
  • oGood IT skills and experience using electronic patient records and Microsoft Office applications.

Desirable

  • oExperience of service development or change management.
  • oExperience working within acute hospital services.
  • oExperience contributing to policy and protocol development.
  • oExperience leading audits or participating in research activity.
Qualifications and Training

Essential

  • oRegistered Nurse (Level 1), Learning Disabilities, or Registered Nurse with significant experience supporting people with learning disabilities.
  • oDiploma level qualification or equivalent professional education
  • oRecognised teaching or assessing qualification
  • oEvidence of ongoing Continuing Professional Development
  • oWillingness to undertake further training relevant to the role.

Desirable

  • oPost-registration qualification relevant to learning disability practice
  • oDegree level qualification or working towards one
