Job summary

Neighbourhood Advanced Clinical Practitioner: Permanent (30 hours)

Stockport NHS Foundation Trust is delighted to offer an exciting opportunity for an enthusiastic and motivated Advanced Clinical Practitioner (ACP) to join our Neighbourhood Team within the Urgent Neighbourhood Services Directorate.

We are looking for an ACP who holds aMaster's degree in Advanced Clinical Practice (or equivalent) and is passionate about delivering innovative, compassionate care. Main duties of the job

This rewarding community-based role offers the chance to work autonomously as part of a multidisciplinary team, delivering high-quality, patient-centred care to people within their own homes and local communities. Working within your scope of practice and the Advanced Clinical Practice framework, you will provide advanced clinical assessment, diagnosis, treatment and care planning, using expert judgement, clinical decision-making and professional leadership to achieve the best outcomes for patients.

This post is subject to an Enhanced DBS check. Upon appointment, the cost of the check (£54.90) will be deducted from the successful candidates first Trust salary.

Person specification

Education

Essential

  • Professional Registration
  • Non- Medical prescribing course (in eligible professions)
  • MSc in advanced clinical practice

Desirable

  • Teaching or relevant experience qualification
Knowledge & Experience

Essential

  • Advanced clinical practice skills
  • Evidence of research and audit
  • Evidence of implementing change
  • Evidence of dissemination of knowledge in wider healthcare organisations
  • Experience Post registration experience at a senior level (band 7 or above) in community or a relevant speciality area
  • Evidence of on-going CPD
  • Evidence of teaching in a variety of environments
  • Experience of using multidisciplinary approach to patient care
  • Ability to work independently.
