Job summary
Care Plus Group Primary Care practices provides the first point of contact for registered patients accessing the health care service. The service will work collaboratively with patients to assist them in managing and preventing illnesses and coordinating access to specialised care.
The Care Plus Group Primary Care practices, Open Door and Quayside, provide services for a client group with a range of diverse and sometimes complex needs including substance misuse, homelessness, mental health problems and self-neglect.

Job Title: Nurse Associate
Hours: 30 hours per week, Mon - Fri
Based at: Open Door and Quayside Medical Centre
Permanent Contract
£29,110 pro rata per annum,fixed salary point
An exciting opportunity has arisen for a Nurse Associate to join our expanding team within CPG Primary Care providing services for a client group with a range of diverse and sometimes complex needs including substance misuse, homelessness, mental health problems and self-neglect.
You will:
- Work under the direction of the practice clinical team/ Advanced Nurse practitioner or in supporting other Health and Social Care Professionals and the delivery of Health and Social Care interventions to Service Users.
- Be expected to use their own initiative within their area of competence.
- Be a qualified Nurse Associate and be registered with the NMC and have experience of working within a community setting and undertaking a range of clinical skills, In particular complex wound care, patient health checks, venepuncture and ECG.

Qualifications
Essential
- Registered Nursing Associate on the NMC register
- Educated to a minimum level 5 Foundation degree level specific to nursing associate practice
- Care Certificate or willing to achieve within 6 months of commencement in post
Skills & Knowledge
Essential
- Ability to work autonomously in the community setting
- Effective interpersonal and communication skills
- Ability to organise and prioritise own workload
- Able to recognise own limitations in the context of the nursing associate role
- Know when to seek advice and refer to a registered health or care professional
- Ability to participate in reflective practice and clinical supervision activities
- Basic computer skills
- Knowledge of Systmone computer system including mobile version
- Able to demonstrate knowledge and skills to others
- To have a range of clinical skills, such as, wound care, eye care, pressure area care, venepuncture, bowel/bladder care, clinical observations including blood pressure and blood glucose monitoring
- Ability to maintain clear and accurate electronic and paper records

- Knowledge and understanding of the regulations relating to Health and Social Care
