Job summary

As a Practice Nurse, you will play a key role in managing the care of patients with long-term conditions and contributing to preventative and proactive health strategies. This varied and rewarding role includes.

Providing evidence-based care to patients with chronic conditions (e.g., asthma, diabetes).

Delivering wound care, cervical screening, immunisations and phlebotomy.

Supporting health promotion initiatives and travel medicine.

Conducting patient assessments and managing treatment plans.

Liaising with the multidisciplinary team to support patient wellbeing.

Ensuring compliance with infection control and safeguarding standards

Main duties of the job

Lead or contribute to chronic disease management clinics.

Provide vaccinations to both adults and children.

Deliver women's and men's health services.

Undertake wound care and routine nursing procedures.

Support the management of frail or housebound patients.

Participate in audits, policy development, and clinical improvement initiatives.

Maintain accurate clinical documentation and coding SNOMED CT.

Qualifications

Essential

  • Registered Nurse with valid NMC registration
  • Diploma or Degree in Nursing
  • Evidence of continued professional development (CPD)
  • Experience of working in primary care or community settings
  • Experience of cervical cytology, immunisations, and wound care
  • Experience of working independently and as part of a team
  • Knowledge and Skills
  • Knowledge of chronic disease management protocols
  • Skilled in venepuncture, vaccinations, ECGs, dressings
  • Ability to assess, plan, implement and evaluate care
  • Understanding of infection control and safeguarding procedures
  • Excellent interpersonal and communication skills
  • Caring, compassionate and professional manner
  • Flexible and adaptable approach to work
  • Commitment to patient-centred care
