Job summary

An exciting opportunity has arisen for an experienced Practice Nurse to work here at Walderslade Surgery. Working 24 hours per week over Wednesday, Thursday & Friday.

We are looking for a candidate, full of enthusiasm and ready for a challenge. The role will be varied and cover all aspects of Practice Nursing.

Main duties of the job

We are looking for an experienced Practice Nurse in Chronic Disease Management who is friendly and enthusiastic to provide a high quality of care to our patients. At Walderslade, we comprise of 4 GP partners, 5 salaried GPs, an excellent nursing team of Practice Nurses and Health Care Assistants, and a supportive and experienced administration team.

We use EMIS Web as our clinical system.

Job responsibilities

The Practice Nurse will work as part of our primary healthcare team providing care to the patients of Practice

  • Monitoring and management of chronic disease areas such as Asthma, COPD, hypertension, diabetes, further training may available if required in specific areas.
  • Cytology smear taking, swabs, basic sexual health & contraception
  • Conducting investigations, ECGs, 24 BP recordings etc.
  • Immunisations including baby and travel vaccinations.
  • Dressings and treatment room procedures.
  • Requesting pathology tests, for example urine culture, swabs
  • Following agreed clinical protocols with referral to GPs as appropriate
  • Other tasks will include:
  • Following all infection control procedures and ensuring everything is in place, regular checks etc.
  • Chaperoning and assisting patients who are being examined by another clinician.
  • Participation in administrative systems in the Practice
  • At all times there will be a need to maintain accurate records
  • A duty to advise GPs of potential problems or errors within the range of assigned tasks
  • Attend and participate in any Practice meetings when required.
  • On occasion there may be a requirement to undertake home visits
  • Any other delegated duties appropriate to the post

This role will support the wider team with:

  • Clinical audits
  • Safeguarding concerns
  • Support and participate in shared learning over the Practice
  • Support for GPs in patient care
  • Ability to review clinical practices and respond to national policies and initiatives where necessary
  • Participate in local initiatives in order to enhance local patient care where necessary
  • Monitoring of safe storage, rotation and disposal of medicaments.

Person specification

Experience

Essential

  • Experience of working in a primary care environment
  • Experience of working as a practice nurse or community nurse
  • Experience of chronic disease management
  • Experience of COPD, Asthma management
Qualifications

Essential

  • Registered Nurse - with valid NMC Registration

Desirable

  • Post graduate diploma or degree (Primary Care)
