In this role, you will provide safe, effective care within the scope of practice nursing, including:
Longterm condition management
Health promotion and disease prevention
Screening activities
Supporting patients to stay well and manage their health confidently

Main duties of the job
Clinical CareProvide evidencebased care for patients of all ages.
Deliver longterm condition management including asthma, COPD, diabetes, hypertension, etc.
Carry out cervical cytology, immunisations, wound care, ear irrigation, ECGs, and phlebotomy.
Support urgent care through assessment, triage, and treatment planning within scope.
Promote health education, lifestyle change, and selfmanagement.
Recognise deterioration, interpret findings, and escalate appropriately.
Maintain accurate clinical records in line with NMC and organisational standards.
Work in accordance with the NMC Code professional standards.
Follow policies for safeguarding, infection control, medicines management, and governance.
Contribute to audits, quality improvement, and development.
Maintain required competencies, mandatory training, and safeguarding levels.
Support development of clinical pathways, protocols, and bestpractice guidance.
Work effectively with GPs, ANPs, HCAs, ARRS roles, and administrative teams.
Support induction, supervision, and development of junior staff and HCAs.
Participate in MDT meetings, case discussions, and PCN initiatives.
Provide clinical advice within scope of practice.
Build positive, supportive relationships with patients and carers.
Communicate complex/sensitive information clearly and compassionately.
Promote shared decisionmaking and personalised care.
Ensure patients understand treatment plans

Job Statement
The Practice Nurse delivers highquality, patientcentred nursing care within the primary care environment. The role combines autonomous clinical practice with collaborative multidisciplinary working, supporting longterm condition management, preventative health interventions, urgent care, and treatmentroom services. The postholder contributes to safe, effective, and compassionate care while supporting the operational and clinical priorities of the practice and wider PCN.
Key Responsibilities
Clinical Care
- Provide evidencebased nursing care to patients across the lifespan.
- Deliver longterm condition management (e.g., asthma, COPD, diabetes, hypertension, CVD).
- Undertake cervical cytology, immunisations, wound care, ear irrigation, ECGs, and phlebotomy.
- Support urgent care through assessment, triage, and treatment planning within scope.
- Provide health promotion, lifestyle advice, and selfmanagement support.
- Interpret clinical findings and escalate concerns appropriately.
- Maintain accurate, timely clinical records in line with NMC and organisational standards.
Professional Leadership & Governance
- Work in accordance with the NMC Code and professional standards.
- Adhere to safeguarding, infection prevention and control, medicines management, and clinical governance frameworks.
- Participate in clinical audits, quality improvement, and service development.
- Maintain uptodate competencies and mandatory training, including safeguarding levels 13 as required.
- Contribute to the development of clinical pathways, protocols, and best practice guidance.
Teamwork & Collaboration
- Work closely with GPs, ANPs, HCAs, ARRS roles, and administrative teams.
- Support induction, supervision, and development of junior nursing staff and HCAs.
- Participate in MDT meetings, case discussions, and PCNwide initiatives.

Experience
Essential
- Minimum 1 years post registration experience
- Experience in a primary care, community, or similar clinical setting.
- Competence in core treatment room skills.
- Experience managing long term conditions.
- Experience working autonomously within scope of practice.

Qualifications
Essential
- GCSE grade A to C in English and Maths
- Registered Nurse
- Evidence of ongoing professional development.
- Immunisation and vaccination training.
- Cervical cytology training (or willingness to undertake).
- Safeguarding training appropriate to role (Levels 13).
Desirable
- Evidence of appropriate knowledge base
- Broad knowledge of clinical governance
- Chronic disease management qualifications (e.g., asthma, COPD, diabetes).
- Mentorship or supervision qualification.
- Additional primary care related certifications.
