Job summary
This is an exciting opportunity for an experienced Practice Nurse at our surgery, who will play an important role in supporting patients with chronic and long-term conditions, with a particular focus on diabetes. We are looking for a Practice Nurse, who is confident in managing these conditions and is keen to develop their knowledge and skills within primary care.
Main duties of the job
The right candidate will provide a broad range of nursing services, including long-term condition reviews, health promotion and preventative care. Clinical duties will include observations and investigations, wound care, vaccinations and immunisations, cervical screening, travel health and other procedures appropriate to their training and competence.
The Practice Nurse will work closely with GPs and the wider multidisciplinary team, while also managing their own workload and working independently within their scope of practice.

Job responsibilities
The appointed Practice Nurse will maintain accurate clinical records, follow safeguarding, infection prevention and control procedures, and comply with Practice policies and relevant guidance. The role will also involve supporting colleagues and students, contributing to clinical quality standards, maintaining NMC registration and undertaking continuing professional development.
A key part of the role will be the routine review and monitoring of patients with diabetes. This will include reviewing blood pressure, weight, BMI and other relevant clinical measurements, as well as providing advice on diet, exercise and self-management. The Practice Nurse will identify patients requiring further assessment, medication review or referral and ensure care is delivered in line with current clinical guidance and practice protocols.

Qualifications
Essential
- Registered Nurse with current NMC registration.
- Experience of assessing and managing patients with long-term conditions.
- Experience of, or a strong interest in, diabetes care and management.
- Ability to work autonomously and as part of a multidisciplinary team.
- Good clinical assessment, decision-making and organisational skills.
- Excellent communication and interpersonal skills.
- Commitment to delivering compassionate, patient-centred care.
- Understanding of safeguarding, infection prevention and control, and professional standards.
- Commitment to continuing professional development and maintaining up-to-date clinical knowledge.
- Experience in primary care or community nursing would be essential, as would previous experience in diabetes care, structured reviews or chronic disease management.
- Experience of cervical cytology and supporting patients with lifestyle and behaviour change would also be essential.
Desirable
- The appointed candidate will have recent nursing experience and be able to demonstrate experience of working effectively within a multidisciplinary team.
- Experience of working within a GP Practice, using clinical systems such as EMIS and contributing to QOF or other clinical quality targets would be advantageous.
