Job summary

Meadowgreen Health Centre is a multi-site GP organisation committed to delivering excellent, patient-centred care across a diverse population. We are currently looking for a motivated full time Practice Nurse to join our friendly and forward-thinking nursing team. We would consider applications from part-time candidates with the relevant experience and skills.

You'll be supported by an experienced leadership structure and have access to regular training, clinical supervision, and the opportunity to take ownership of specific clinical areas.

Main duties of the job

Candidates would preferably have experience in:

  • Childhood and routine immunisations
  • Holistic care of patients
  • Advising on and administering vaccinations
  • Cervical cytology screening
  • Wound care management
  • Contraception advice and reviews
  • Travel advice and vaccinations

  • V300 non-medical prescribing desirable

About us

We are a 19,000 patient, 2 site practice in the south of Sheffield with a wide ranging demographic. We have 7 GP Partners, 11 Salaried GP's, 8 Practice Nurses, 3 HCA's & 2 Phlebotomists in addition we have a large reception & admin team. We are part of Peak Edge PCN with a pharmacy and social prescribing team.

Job responsibilities

Accountability: Reports to the Lead Practice Nurse for day-to-day management, Practice Partners for clinical and professional matters, and the Practice Manager for employment issues.

Purpose of the Role: To provide high-quality, patient-centred nursing care, supporting the assessment, treatment and management of patients with acute, chronic and long-term health conditions. The role promotes health and wellbeing, disease prevention and self-management, working collaboratively with the multidisciplinary team to deliver safe, effective and evidence-based care.

Key Responsibilities:

  • Assess, plan, deliver and evaluate care for patients with acute, chronic and complex health needs.
  • Undertake health promotion, screening, immunisations, chronic disease monitoring, wound care and medication reviews within scope of practice.
  • Support patients to improve health outcomes
  • Recognise and respond appropriately to urgent clinical situations, referring when required.
  • Communicate effectively with patients, carers and healthcare professionals to ensure coordinated care.
  • Maintain accurate clinical records and practise in accordance with NMC standards, national guidance and practice policies.
  • Contribute to audits, quality improvement, safeguarding, infection prevention and risk management activities.
  • Liaise with community and specialist services to support continuity of care.
  • Maintain professional development and support the learning and development of colleagues and students where appropriate.

Person specification

Qualifications

Essential

  • Registration with the Nursing and Midwifery Council.
  • Evidence of ongoing CPD relevant to the role.

Desirable

  • Relevant experience of working in a primary care setting.
  • Experience in audit and protocol development skills, experience and qualifications in: hypertension, asthma and COPD, CVD, Diabetes, child and travel immunisations, contraception, cytology, menopause, health promotion, wound care, shared care drug monitoring.
  • Use of SystmOne clinical system
