Job summary

Guardian Medical Centre is seeking an enthusiastic, experienced and motivated Practice Nurse to join our friendly, dynamic and forward-thinking team.

This is an exciting opportunity for a skilled nurse to become part of a progressive primary care team, working alongside a wide range of healthcare professionals to provide excellent clinical care to our patient population.

Main duties of the job

Main Duties of the Role

The successful candidate will be responsible for delivering a high standard of nursing care and supporting the management of patients with both acute and long-term health conditions.

Essential experience includes:

  • Childhood immunisations and vaccination programmes
  • Cervical screening

Chronic disease management, including:

  • Asthma
  • Diabetes
  • COPD
  • Administration of injections and vaccinations
  • Health promotion and patient education

The successful candidate will be expected to work autonomously whilst also contributing positively as part of the wider practice team.

Job responsibilities

Key Responsibilities

Clinical Duties

  • Deliver high standards of nursing care within professional competence and scope of practice.
  • Undertake comprehensive patient assessments and provide appropriate treatment, advice and referrals.
  • Deliver and support vaccination and immunisation programmes in accordance with national and local guidance.
  • Administer prescribed medications, injections and vaccinations under Patient Specific Directions (PSDs), Patient Group Directions (PGDs) or independent prescribing qualifications where appropriate.
  • Undertake cervical screening in line with national screening requirements.
  • Perform NHS Health Checks and health promotion activities.
  • Carry out routine treatment room procedures including:
    • Venepuncture
    • Blood pressure monitoring
    • ECGs
    • Urinalysis
  • Support infection prevention and control standards.
  • Recognise and respond appropriately to safeguarding concerns affecting adults and children.
  • Respond effectively to medical emergencies and participate in resuscitation procedures as required.
Chronic Disease Management
  • Manage long-term conditions through structured reviews and monitoring, including:
    • Diabetes
    • Asthma
    • COPD
    • Cardiovascular disease
    • Hypertension
    • Chronic kidney disease
  • Support patients to self-manage long-term conditions.
  • Monitor treatment effectiveness and escalate concerns to GPs or other clinicians.
  • Participate in care planning and multidisciplinary reviews.
Health Promotion and Patient Education
  • Promote healthy lifestyles and preventative healthcare.
  • Provide advice regarding smoking cessation, weight management, exercise and alcohol consumption.
  • Support national and local public health initiatives.
  • Encourage patient engagement and shared decision-making.
Clinical Governance and Quality
  • Maintain accurate, contemporaneous and confidential patient records using the practice clinical system.
  • Work in accordance with NMC Code, practice policies and relevant legislation.
  • Participate in clinical audits, quality improvement projects and service development activities.
  • Report incidents, significant events and safeguarding concerns in accordance with practice procedures.
  • Support compliance with CQC standards and other regulatory requirements.
  • Maintain professional registration and mandatory training requirements.
Communication and Team Working
  • Work collaboratively with GPs, healthcare assistants, pharmacists, PCN colleagues and other members of the multidisciplinary team.
  • Communicate effectively with patients, relatives, carers and healthcare professionals.
  • Participate in team meetings, clinical supervision and practice development initiatives.
  • Support and mentor healthcare assistants, trainee nurses or student nurses where appropriate.
Professional Responsibilities
  • Maintain registration with the Nursing and Midwifery Council (NMC).
  • Undertake continuing professional development (CPD) and mandatory training.
  • Work within professional competence and seek guidance when required.
  • Adhere to practice confidentiality, information governance and data protection requirements.

Qualifications

Essential

  • Registered Nurse (Adult) with current NMC registration
  • Chronic disease management qualifications
  • Immunisation and vaccination qualification
  • Cervical screening accreditation

Desirable

  • Independent Nurse Prescriber qualification
Experience

Essential

  • Previous experience in General Practice or Primary Care
  • Understanding of long-term condition management
  • Experience of chronic disease review clinics
  • Knowledge of infection prevention and control
  • Understanding of safeguarding responsibilities
  • Excellent communication and interpersonal skills
  • Competent IT skills and experience of electronic clinical record systems

Desirable

  • Knowledge of Quality and Outcomes Framework (QOF)
  • Experience using SystmOne
